Prepare for your visit
If this is your first visit, or if your health history has changed, please fill out this health history form and bring it with you.
Loose, comfortable clothing is best, as Acupuncture therapy involves palpation and treatment of various parts of the body. Draping will be provided to ensure modesty. Please refrain from wearing excessive jewelry, as some metals may interfere with the efficacy of the therapy.
Because we treat patients with allergies, we would appreciate it if you would not wear perfume, cologne, aftershave or scented lotions, etc. to your appointment.
Cold & flu season
There are many children in and out of our office. If your children are sick and you have an appointment, please make arrangements to leave your child at home.
If you need to cancel your appointment, please do so no less than 24 hours prior to your scheduled appointment. Late cancellations and “no shows” will be charged 1/2 the fee for that missed appointment.
Payment is due at the time of service. We gladly accept checks, cash or credit/debit cards.
We do not process Health Insurance. We can supply you with a Super Bill that is signed and coded so you can be reimbursed by your insurance company if they cover Acupuncture.
Acupuncture is a valid medical expense for flex plans and health saving accounts.
